Enter PUK - your SIM cannot be accessed - what is my PUK?

I am having trouble activating my new sim card. It says that my sim is activated and i have gone through all the steps successfully, however, my iphone is saying that i need to enter my PUK code before i can access my sim card. I have no idea what the PUK code is or would be.

I tried 0000 and 1111 and i tried my previous bell pins, and none of them worked.

I could not find a 4 digit code anywhere on the letter i was sent or on my account.
